VERIFICATION OF ENROLLMENT:
To request verification of enrollment for DPS (for driving purposes), please click on the below link.
GENERAL DOCUMENT REQUEST FORM:
To request any other type of document such as report cards, letters, immunization records, etc please click on the below link.
To report any changes in information including phone numbers, home/mailing address, and email address.
To give consent for medication to be administered at school, please complete a MEDICATION CONSENT FORM.
NOTIFICATION OF HIGH SCHOOL COURSE OR EXAM:
Please use the below attached form if you take a high school course or credit by exam outside of Meridian.
